Medical Insurance

Therapy Care accepts many types of insurance plans including PPO, POS, HMO, Worker’s Compensation and Medicare.  We gladly accept the insurance plans listed below.  If your plan is not listed, please call us at 630-761-0900 as we are always adding new insurance plans.

As a courtesy to our patients, we will verify your health benefits to determine if there will be a copay, deductible, or coinsurance for therapy services.

Disclaimer:  Health plans do change regularly without prior notification.  We recommend that you verify with your health plan what therapy benefits you have available.  We have included most of the major plans with which we are in-network. However, this is not an all inclusive list. Please call the office to verify other insurance plans.

Cash Pay Services Pack

High deductible plans are unavoidable these days.  For those families who have high deductible plans or those who have maxed out their benefits under their insurance plan we offer discounted cash pay services packs when a pack of 8-10 weeks of services are pre-paid.  These can be paid for with cash, credit or through CareCredit.

Multiple Services Discount

For families needing more than one type of therapy, Therapy Care provides discounts for therapy when more than one service is received.

CareCredit

Therapy Care offers CareCredit.  CareCredit is different from a regular credit card.  It helps you pay for out-of-pocket expenses not covered by medical insurance by extending special financing options.  In addition to our office, you can use your CareCredit to pay for health, and wellness care at over 200,000 enrolled providers across the nation. Once you've applied, you can use it again and again at any location that accepts CareCredit.

Therapy Care offers short term financing of 12 months with no interest on purchases of $200 or more when you make the minimum monthly payments and pay the full amount due by the end of the promotional period.

Being A Patient With Us

Therapy Care thanks you for being a patient with us!  Below are a number of items which will help you feel more comfortable while under our care. 

 

For Your First Visit

  • Depending on your condition, a typical session will last approximately 30-90 minutes.

  • If possible, bring completed your Patient Forms with you.  If not possible, please arrive 15 minutes before your first appointment to complete the paperwork.

  • During your first visit, your therapist will typically conduct an initial evaluation.  The initial evaluation will consist of subjective and objective findings which will assist in the development of an appropriate treatment plan tailored specifically to you or your child.  At that time your therapist will discuss with you the length and frequency of your rehabilitation treatments and provide you with a home program. A copy of the Initial Evaluation will be sent to your physician.  Together with you and your physician the therapist will determine what goals are to be achieved during therapy.  To ensure clear and concise communication to your doctor and insurance provider, your therapist will use electronic documentation during each visit to document your treatment and progression toward these goals.

  • If you are running late, please call and let us know so that we can accommodate your schedule.

  • Please notify us 24 hours in advance of your appointment if you need to cancel

  • All new patients and clients will be required to provide valid photo identification (Driver’s License, State ID, Passport, School ID) in addition to their insurance card.

  • Upon arriving, we ask that you check in with one of the staff members and have a seat until your therapist comes to meet you.  Please do not begin any activities until you receive instruction from the staff.

  • As a courtesy to other patients, please turn off all cell phones during treatment.

  • All co-pays are expected at time of service.

 

Patient Billing

Once you have scheduled your appointment and have provided insurance information, as a courtesy we will verify your health insurance benefits.  Please note that insurance quotes are not a guarantee of benefits or coverage.  We encourage you to review and confirm your benefits with your insurance company as they may differ from your standard medical coverage.  The following is intended to help you better understand our billing process:

 

  1. Your therapist will forward your daily charge to our billing manager.

  2. Our billing manager will submits these charges to your insurance company for reimbursement.

  3. Therapy Care generally receives payment within 30-60 days.

  4. Therapy Care will provide you with an invoice after your insurance has paid or made a decision on services rendered (usually within 30-60 days).

  5. Although it may take 30-60 days to receive a bill for your deductible and co-insurance, all co-pays are expected at time of service.

Progress Summary Reports

Depending on your insurance and your doctor, your therapist will complete a Progress Summary that will be sent to your physician, so please keep us informed of any upcoming follow-up visits you have with your doctor.  This letter will inform your physician of your progress in therapy and will generally give one of the following recommendations:

 

  • A request for the approval of additional therapy for a specified amount of time.  Generally, your physician signs a prescription that is attached to the Progress Note and returns it to us.  The prescription is necessary in order to continue with care.  As a courtesy to you, we will contact your physician’s office for an additional prescription.

  • A recommendation to discharge you or your child from therapy.  Your physician may choose to discharge you or your child if you have met your goals, are no longer progressing in therapy, or at her/his discretion.
